Why should I refer my patient to Fairfax Cryobank?

How can Fairfax Cryobank assist my patient in donor selection?


Fairfax Cryobank customer service can answer any inquiries that you may have, provide donor availability, and also guide you through the selection process. A patient information packet can be obtained through customer service as well or by completing our online request form. The Fairfax Cryobank Client Services Department provides photo consultation and the donor selection consultation. You'll find that the information you receive using these services is a more comprehensive and in-depth look into particular donors.

What paperwork do I need to complete to open an account?


Fairfax Cryobank requires that health professionals, if he/she has not already, establish an account with us before sperm specimens are ordered and shipped. The form your office must fill out is available by contacting Client Service at 800-338-8407.

How long does it take to open an account?


It takes one business day to open an account from the time we receive your completed form.

 

Does the physician need to sign a consent form before a patient places an order?


Effective November 1, 2006 for new patients, the Terms of Use Letter must be signed by the patient and returned to us prior to shipment of any donor specimens. The form does NOT need the signature of the physician. The form may be returned by fax, mail or a scanned image into an email. Contact Client Service at 800-338-8407 for assistance if needed. The Terms of Use Letter explains what to expect from our semen donor program and what we expect from the patient when purchasing our semen and other services.

Patients ordering ID Options donors need to return a signed and notarized consent before units can be shipped.

 

What information needs to be provided when ordering donor sperm?

 

At the time of your order, you will need to have the following:

  • Health care provider's name, address, and phone number

  • Shipping address (if different than provider's address)

  • Patient name, address, and phone number (or your customer number if you have placed other orders)

  • Donor number(s) in order of preference

  • Number of vials and preparation type (ICI, IUI or IVF)

  • Date the units need to arrive at the provider's office

  • Type of shipping (FedEx Two Day, FedEx Priority Overnight, local courier, patient pick up)

  • Payment information.

  • Signed and returned Terms of Use letter

 

Fairfax Cryobank accepts all major credit cards, money orders and cashier checks. Personal checks are not accepted. Fairfax Cryobank must receive any money order or cashier check before the shipping date. We do not work with or arrange payment with insurance companies, although we can provide the patient with copies of paid invoices to submit to insurance upon request.

Will you ship sperm directly to a patient's home?


Fairfax Cryobank can ship samples to a patient's home once the physician completes the Home Delivery Consent Form. Please contact Customer Service at 800-338-8407 to have the form faxed to your office.

 

Do you provide instructions and materials for home insemination?

Instructions for performing the procedure and equipment should be obtained from the physician's office.

 

What is your thaw procedure?


The thaw procedure is specific to the unit. Unit specific instructions are included in every shipment. Please click here to download detailed thaw procedures.

 

What are your minimum standards for specimens?

ICI and IUI-ready specimens must meet the minimum post thaw standard. Please also see full details of our Specimen Quality Standard.

How do the specimens arrive?


All of our specimens are shipped in liquid nitrogen vapor tanks via FedEx, local courier, or local patient pick up. The liquid nitrogen vapor tank is inside of a hard outer case for FedEx orders and a cardboard box for local orders. The inner canister (vapor tank) is fastened using a cable tie to prevent tampering. Shipping papers are contained inside the outer case which contains a packing slip with the following information about your order: your health care provider's name and address, your name and address, your donor number and specimen preparation, number of vials, and post thaw count and motility for each units. Also included are a summary of records detailing the donor testing and a statement of donor eligibility, specimen thawing instructions, return instructions and prepaid return labels.

 

How long do the specimens remain frozen in the tank?


Our standard shipping tanks have a guarantee of 7 days. Day 1 is considered the day of shipment (not arrival). Upon request and for an additional fee we can send the specimens in a 2 week tank in which the vials are guaranteed frozen for 14 days.

 

How do I return the shipping container?


Please contact FedEx Ground by calling 1-888-777-6040 or 1-800-GO-FEDEX.

 

What do I do if I lose the return label for the container?


Contact Client Services (1-800-338-8407) and a replacement label will be sent.

What if we need to return the specimens?


We realize that situations may occur in which the return of the frozen specimens is necessary. We will accept specimens returned to us providing (1) we are promptly notified that they are being returned, (2) the specimens have not been removed from the shipping container and (3) the shipping container is received by Fairfax Cryobank within 7 days from the initial shipment date. The return shipping documents, which are included in the shipment, require 2 days to return the shipment to Fairfax Cryobank. If the return shipment must be shipped via Overnight Service in order to comply with the 7 day time limit, the clinic/patient must make their own shipping arrangements including payment of fees. A shipping credit will not be issued. The returned specimens will be maintained in storage for future use by the same patient. We cannot, and will not, use these specimens for any other physician or patient. The clinic/patient is responsible for payment of the original invoice.

 

What type of storage services do you offer if the patient wants to purchase and store extra semen specimens?


Fairfax Cryobank offers storage terms for storage accounts ranging from 1 month to yearly long-term contracts. Purchasing units to put into a storage account is a great way to ensure that vials from the donor you have selected are available for both current and future cycles. Do you offer a Buy Back program?

Fairfax Cryobank started a Buy Back program in 1990.You may return any unused specimens that were purchased at regular prices and have never left the Fairfax Cryobank facility and receive half the original purchase price back. This includes ANY specimen purchased at ANY time. There are also NO limits to the number of vials you can return for this partial refund. There are NO RESTRICTIONS regarding the status of a client's account. Please note that our Vial Buy Back program is not offered on specimens that have been transferred by the original owner. A major west coast cryobank has a vial buyback program as well, but there are several important differences. That cryobank's program started in August of 2005. Only vials purchased after this date are eligible for a 50% of purchase price buyback. Its program has LIMITS to the number of vials that apply and the repurchase must occur BETWEEN 6-24 MONTHS after the original purchase. Also, RESTRICTIONS apply to the status of client accounts. Fairfax Cryobank's is a simpler and clearly superior deal: If you want to return vials, Fairfax Cryobank will repurchase them at 50% of their original selling price. We have no strings or conditions except the vials cannot have left our facility.

What quality control measures do you take to ensure that the client is receiving the correct semen specimen?

 

See our policy on verifying shipments. Fairfax Cryobank's quality control measures include:

  • A Fairfax Cryobank staff person verifies the donor number and donor description with each client at the time the order is placed.

  • Computerized specimen selection with donor number, unique identifier and matching tank location.

  • Two cryogenic technicians independently check each shipment.

If the quality (sperm number in specimen) is inadequate after thawing, what steps should the patient/clinic take?


If the specimen quality is inadequate after thawing, the patient/clinic should contact our Complaint Department at 703-698-3961 and the person who determined specimen quality must complete and fax us the form at 703/245-7074.

 

May the patient speak with a staff member who has worked with the donor?


Due to many requests from patients to talk to a staff member who has worked with a particular donor we now offer Staff Impressions free of charge through our donor search feature. Laboratory staff members who have worked closely with the donors write staff impressions. Donors who do not have staff impressions available are no longer active in our program and we do not have staff members familiar with the donors.

 

Can Fairfax Cryobank provide genetic counseling to the recipient to guide her in donor selection?


Some of our clients may have special circumstances that require them to select donors tested for a specific genetic disease that runs in their family or simply to gather more information about a genetic or hereditary situation. Help is available from our genetic counseling staff. Consultations by appointment are available.

 

Should my patient select a CMV negative donor?


Fairfax Cryobank urges you to discuss the issues of CMV with your patient. After all, you are in the best position to explain how the issue pertains to her specific situation. Please see our CMV information page for more details on CMV.
